High Point Treatment Center Selects Fivebase
Hampton, NJ—High Point Treatment Center in New Bedford, Massachusetts, which provides substance use disorder and mental health services to individuals, families, and communities, has selected Fivebase as its technology partner to develop and share the necessary paperwork and consent forms through a secure portal, allowing clients and guardians to complete and electronically sign them.
Fivebase provided High Point Treatment Center with an electronic signature solution that directly integrates with their Avatar EHR system. The platform enables patients to receive, sign, and return confidential documents through secure SMS/email links, with signed documents automatically updating patient charts without staff intervention. The system features automated reminders, audit trails, and can expand to include additional patient portal services like appointment scheduling and bill payments.
